Holzer Post-Acute Care Services is a member of the Alternate Solutions Health Network. The Alternate Solutions Health Network collaborates with health systems in joint venture partnerships to create a post-acute care solution, delivering efficient centralized operations and utilizing a best-in-class software platform.

Holzer came to Forts Studios through our preferred commercial printer. They were in need of a wall-mounted poster to serve as a sales-tool for their home care services. The goal was to create a graphic that displayed their four core service offerings – skilled nursing care, occupational therapy, home health aide services and physical therapy – in addition to a map of their coverage area.

For this project I decided to go with more of an infographic style. The coverage area is placed inside a circle at the center of the design like a hub. Each service is linked back to the center to show how everything is interconnected. There is minimal text, and it is done in an easy-to read font to grab a viewer’s attention quickly. Last but not least, we kept the color palette simple using Holzer’s existing branding of blue and white.

The end result is an eye-catching poster that serves as a great sales tool when mounted in their customers’ offices.
